Iâve got to be honest. . .I, like many new nurses struggled A LOT with charting. Getting it done fast and accurately was really hard for me.The cheatsheet for today is going to give you some quick tips on charting more effeciently.A quick additional note about head to toe assessments. Â As mentioned above, your goal in the assessment is to find abnormalities. Â If you think about it this way you will find increased confidence and speed in your assessments. You arenât looking over every strand of hair on their head, nope, you are looking to see if they have hair growth, if their hair is cared for, if there are any scars of wounds on the head. . . do you notice anything that SHOULDNâT be there or are they missing something that SHOULD be there? Â Do any of those findings correlate to disease processes?I know assessing patients for the first time can be intimidating. Â With time you will come to enjoy them and actually start to think of them as little investigations . . . what will you uncover???Hope this helps!
